This video helps teams understand what needs to be in place before assessing readiness and sets up teams for success as part of a change and implementation process at a child welfare agency.

Presents the fifth task, documenting assumptions and rationale, and provides examples of how to apply strategies. A clear, concise narrative can help to engage stakeholders, decision makers and potential funders in future steps to address the root cause and problems identified.

Presents the third task of collecting and analyzing data identified in the data plan to help answer research questions. Examining data helps teams tell a more complete story and understand the scope and impact the problem is having on the community.

Presents the sixth and final task of team building: debriefing, evaluating, and identifying the next steps to assess completion and determine how the team will sustain the intervention.
