While preparing for the Child and Family Services Review (CFSR), child welfare agency staff and their partners can use this Systemic Factor Data Planning Worksheet to plan for assessment of how well their statewide information system meets federal requirements. A well-functioning statewide information system houses valid and accurate data and supports the agency in delivering appropriate services to children and families, monitoring safety, and conducting continuous quality improvement activities.

Complete the worksheet’s fillable tables to:

  • Document available data sources that can be used to respond to Statewide Assessment questions and demonstrate systemic factor functioning
  • Plan to fill identified data gaps and address data limitations
