While preparing for the Child and Family Services Review (CFSR), child welfare agency staff and their partners can use this Systemic Factor Data Planning Worksheet to consider evidence of agency responsiveness to the community. Agency responsiveness is achieved through meaningful engagement of individuals with a vested interest in the child welfare system—including young people and families, tribes, caregivers, legal and judicial communities, service providers, and other partners—and coordination of federal child and family services.

Complete the worksheet’s fillable tables to:

  • Document available data sources that can be used to respond to Statewide Assessment questions and demonstrate systemic factor functioning
  • Plan to fill identified data gaps and address data limitations

The tool draws from and should be used together with the more comprehensive information presented in Assessing Systemic Factor Functioning Using Data and Evidence. After completing this worksheet, use similar worksheets to identify data sources for other systemic factors.
